Walk-ins are always welcome. Appointments are not needed for urgent care patients, we will see you when you need to be seen.

For primary care patients in Chicago, please schedule an appointment here, or call us at 773-754-3500.

We accept most insurance plans. Please contact your health insurance provider or contact our clinic if you are unsure.

Alliance Immediate and Primary Care accepts standard payments of cash or checks. We also accept Visa, MasterCard, Discover, and American Express credit cards.

Please call your PMD for advice. If you have a medical emergency, call 911 or go to the nearest emergency room.

We adhere to HIPAA Guidelines, which is a federal law that very carefully protects your private healthcare information. It is our every intention to keep your personal information private. We normally forward information and lab results from your visit to your PMD to assist them in your care. If there some reason you don’t want your record sent to your personal physician please let us know.

Please bring your insurance card and any other medical information (medications, old EKG’s, radiology reports) that may be helpful. We recommend that you maintain an updated list of your medications and healthcare history at www.google.com/health.

Alliance Immediate and Primary Care will provide you a prescription. You may take this prescription to Value Care Pharmacy next door, or another pharmacy of your choosing.

We need to examine you in order to advise you on your illness and provide proper treatment. For established patients, some insurance plans will allow an internet consultation.
